Smith settled in London in 1820 after ministering in Bristol and getting an MD from Edinburgh University. He became part of Jeremy Bentham's circle (he kept Bentham's skeleton in his rooms until passing them to University College, London in 1850) and was published regularly on medical matters with a particular interest in fever.

Smith is prefaced by both Southwood and Southwold in the diary. The Metropolitan Magazine also refers to him as Southwold while other contemporary publications refer to him as Southwood so it is clear that both spellings refer to him. 'sd Smith' and 's smith' (1835) have also been attributed to him.

It is possible that 'rev. T. Smith' mentioned in 1809 also refers to this Smith but this has not been coded.
